How it Works:
- Sign Up: Fill out a quick form, and we’ll call you to see if you qualify.
Fill out the form - Pick Your Food: If you qualify, you can choose from a variety of healthy foods that fit your needs and tastes. (If you don’t qualify, we’ll connect you with other resources!)
- Get Your Delivery: Your food box will be delivered right to your home — all paid for by Medicaid.

- Diet Survey: Tell us about your dietary needs and food preferences.
- Basic Health Information: This helps us match you with meals that support your health goals.
How long does it take to get started?
It usually takes just a few minutes to fill out the short form. Once you complete the diet survey and screening with your care navigator, it may take a few days to process your information and get your first meal delivery.
What kind of meals will I get?
You’ll receive food that fits your health needs, dietary restrictions, and preferences. Depending on your benefit, your box may include:

- Fully Prepared Meals: Ready-to-eat meals that just need heating.
Can I sign up for someone else (like my child or parent)?
