EngageWell’s 20 member organizations are unified in their commitment to work with Medicaid managed care plans, accountable care organizations, large health systems and other IPAs to:
Engage marginalized populations—people with HIV/AIDS, active substance users, individuals who are homeless or unstably housed, and others—into integrated service pathways that will advance the quality and reduce the overall cost of their health care.
Make services more accessible, coordinated and effective by addressing social determinants of health so people who receive care from the EngageWell IPA have longer, healthier and more stable lives.
Reduce health care disparities by eliminating barriers to accessing quality medical and behavioral care for low-income New Yorkers that are associated with race, sexual orientation, poverty, HIV/AIDS, ongoing substance use and/or lack of stable housing.
EngageWell and its network partners are committed to using their skills and experience from addressing the HIV Epidemic to:
Develop accessible and effective interventions to address the Opioid Epidemic.
Offer low-threshold services that reduce health risks and improve engagement in treatment for those actively using substances.
Address the complex medical, behavioral and social needs of people who are homeless.
Provide new ways for low-income clients to access care through a technology-based pilot program
