Applicability and Purpose
EngageWell Community Health & Innovation Collaborative, Inc. (“EngageWell CHIC“) is a nonprofit organization dedicated to advancing health equity through innovative healthcare, behavioral health, and health-related social needs programs.
EngageWell IPA, LLC (“EngageWell IPA“) is a wholly owned subsidiary of EngageWell CHIC and serves as a network of nonprofit health and human service providers working together to improve access to coordinated, person-centered care for underserved communities throughout New York.
EngageWell CHIC and EngageWell IPA operate in coordination while maintaining appropriate corporate separateness. Throughout this Privacy Policy, “EngageWell,” “we,” “our,” or “us” refers collectively to both organizations.
This Privacy & Data Use Notice explains how EngageWell collects, uses, shares, and safeguards personal information and protected health information (“PHI”) in connection with our programs, services, website, and communications.
1. Purpose of This Notice
EngageWell supports health and human service providers in advancing care navigation, care coordination, digital health, quality improvement, technical assistance, and access to evidence-informed health programs.
EngageWell serves as a program administrator, network convener, evaluator, funder, technical assistance provider, and care coordination partner. While EngageWell generally does not provide direct clinical or medical services, we may provide non-clinical client navigation, digital literacy support, health education, and related services in coordination with participating providers and program partners.

Depending on the program or service, EngageWell may collect or receive:

- Demographic information
- Program identifiers
Health Information (Protected Health Information)
Where permitted by law or contract, EngageWell may receive or process health-related information, including:

Digital Health and Program Partners
Technology vendors, telemedicine providers, food and nutrition partners, treatment adherence platforms, evaluation partners, and other organizations contracted to support EngageWell programs.

4. How We Use Information
EngageWell uses information solely for mission-related and program purposes, including:

- With contracted vendors that provide digital health, nutrition, telemedicine, care management, evaluation, or other services supporting EngageWell programs.
- With evaluation and research partners operating under appropriate Business Associate Agreements (BAAs), Data Use Agreements (DUAs), or similar contractual protections.
- With managed care organizations, government agencies, or funders when required by law or contract.
- With service providers performing administrative or operational functions on EngageWell’s behalf.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties
All organizations receiving information from EngageWell are contractually required to safeguard information and comply with applicable privacy, confidentiality, and security requirements, including HIPAA where applicable.
6. Data Security
EngageWell maintains reasonable administrative, technical, and physical safeguards designed to protect personal information from unauthorized access, disclosure, alteration, or destruction.
These safeguards include secure information systems, encryption where appropriate, access controls, workforce training, and periodic reviews of our privacy and security practices.
Although no system can guarantee absolute security, EngageWell continually works to protect the confidentiality and integrity of information entrusted to us.
7. Data Retention
EngageWell retains information only as long as necessary to fulfill programmatic, contractual, legal, audit, regulatory, and evaluation requirements.
When information is no longer required, it is securely deleted, destroyed, or de-identified in accordance with applicable laws and organizational policies.

Because EngageWell frequently acts on behalf of participating providers, managed care organizations, government agencies, and funders, requests may be subject to contractual or legal limitations.
