EngageWell Privacy Policy & Data Use Notice

Applicability and Purpose:

EngageWell IPA, LLC (“IPA”) and EngageWell Community Health and Innovation Collaborative, Inc. (“EngageWell CHIC”) are affiliated entities that operate in coordination to support health and human service providers in advancing care navigation, care coordination, quality improvement, and access to evidence-informed health programs.

EngageWell CHIC serves as the membership-based network organization and employs staff who provide non-clinical client navigation, digital literacy support, health education, quality improvement, and program management functions. EngageWell IPA is the sole-owned subsidiary of CHIC and serves as the contracting entity for managed care, value-based, and other healthcare-related agreements.

This Privacy & Data Use Notice applies collectively to both entities (together, “EngageWell”), which operate in coordination while maintaining appropriate corporate separateness.

1. Purpose of This Notice

EngageWell is a membership-based organization that supports health and human service providers in advancing care navigation, care coordination, and access to evidence-informed health programs. This Privacy & Data Use Notice explains how EngageWell collects, uses, shares, and safeguards personal information and protected health information (“PHI”) in its role as a program manager, funder, coordinator, and evaluator. EngageWell does not provide direct clinical or medical services to clients, but may provide non-clinical client navigation, digital literacy support, health education, and technical assistance in coordination with participating providers and program partners.

2. How EngageWell Receives Information

EngageWell receives information through:

  • Participating Providers: Member health and human service providers that identify clients who may benefit from EngageWell-supported programs, pursuant to Participating Provider Agreements (PPAs) and, where applicable, Business Associate Agreements (BAAs).
  • Third-Party Vendors and Partners: Digital health, food and nutrition, telemedicine, treatment adherence, and other program vendors that support outreach, enrollment, service delivery, and evaluation under contract with EngageWell.
  • Limited Direct Collection: In some cases, EngageWell may collect information directly from individuals (e.g., surveys, outreach, or quality improvement activities) consistent with applicable agreements and laws.

3. Types of Information Collected

Depending on the program, EngageWell may receive or process:

  • Personal Information: Name, contact information, demographic data, and program identifiers.
  • Health Information (PHI): Information related to health status, diagnoses, clinical indicators, dietary needs, or outcomes, as shared by providers, payers, or vendors.
  • Claims and Utilization Data: Healthcare claims, encounters, or utilization data received from managed care organizations or other payers pursuant to value-based or managed care contracts, used solely for evaluation, quality improvement, utilization analysis, and reporting.
  • Technical Information: Limited technical data related to communications or website use.

4. How Information Is Used

EngageWell uses information solely for mission-aligned and program-related purposes, including:

  • Program administration and coordination on behalf of participating providers
  • Supporting outreach, education, enrollment, client navigation, digital literacy support, and technical assistance related to EngageWell-funded programs
  • Quality assurance, quality improvement, and performance monitoring
  • Program evaluation, research, and reporting to funders and partners (often in aggregated or de-identified form)
  • Compliance with legal, regulatory, and contractual obligations

EngageWell does not sell personal information and does not use information for marketing unrelated to program participation.

5. Information Sharing

EngageWell shares information only as necessary and permitted under applicable agreements and law, including:

  • With participating providers, for care coordination and program oversight
  • With third-party vendors, to support outreach, enrollment, service delivery, and evaluation in free programs
  • With evaluation and research partners, such as data analytics or academic institutions, under appropriate data use agreements
  • No mobile information will be shared with third parties/affiliates for marketing/promotion of programs that cost money or with third parties/affiliates unrelated to EngageWell programs or evaluation.

All partners are required to safeguard information and comply with applicable privacy and security requirements, including HIPAA where applicable.

6. Data Security

EngageWell maintains administrative, technical, and physical safeguards to protect information, including access controls, secure systems, and periodic reviews of data practices.

7. Data Retention

Information is retained only as long as necessary to fulfill programmatic, legal, audit, and evaluation requirements. When no longer needed, data is securely deleted or de-identified.

8. Individual Rights and Questions

Because EngageWell acts on behalf of participating providers and funders, individual rights regarding access, correction, or deletion of information may be subject to legal and contractual requirements. 9. Updates to This Notice

This Notice may be updated periodically to reflect changes in programs, law, or data practices. Updates will be posted or otherwise made available as appropriate.

EngageWell Communications & Texting Notice

1. Purpose of This Notice

This Communications & Texting Notice explains how and why EngageWell may contact individuals by phone, text message, email, or other electronic means in connection with EngageWell-supported programs.

2. Why You May Hear From EngageWell

EngageWell may contact you because:

  • Your healthcare or human service provider participates in EngageWell-supported programs; and
  • Your provider identified you as someone who may benefit from program-related education, outreach, enrollment support, or care coordination.

EngageWell conducts these communications on behalf of participating providers and in coordination with approved program partners.

3. Types of Communications

Communications may include:

  • Program information and educational messages
  • Outreach related to eligibility, enrollment, or participation
  • Appointment or enrollment reminders
  • Program-related follow-up and support
  • Support from EngageWell Client Navigators related to using digital tools, understanding program materials, health education, and accessing program-related resources

EngageWell does not send marketing messages unrelated to program participation.

4. Consent and Authorization

Consent to be contacted, including by text message, is obtained through:

  • Your participating provider’s standard consent and care coordination processes; and/or
  • Enrollment and opt-in consent processes administered by third-party program vendors.

When EngageWell sends text messages, it does so for program-related purposes and includes opt-out instructions.

5. Opting Out of Communications

You may opt out of text messages at any time by replying STOP to a text message. You may also contact info@engagewellipa.com with questions about communications.

Opting out of communications may limit EngageWell’s ability to support outreach or enrollment into certain programs but will not affect your eligibility for care from your provider.

6. Changes to This Notice

This Notice may be updated from time to time to reflect changes in communications practices or legal requirements.
